As required by SARA TITLE III

In compliance with EPA's regulation under Section 103(a) of the Comprehensive Environmental Response Compensation Liability Act (CERCLA) and the Emergency Planning and Community Right-To-Know Act (EPCRA), the following steps must be followed in case of SPILL, TOXIC OR HAZARDOUS SUBSTANCE RELEASE, sudden or planned, FIRE OR EXPLOSION on the main campus of ÐãÉ«¶ÌÊÓƵ.

There are four groups of chemicals subject to reporting:

  1. Extremely Hazardous Chemicals (about 300). . . 301-304

  4. Toxic Chemicals (about 320). . . 313

EMERGENCY NOTIFICATION PROCEDURES

The person who discovers the above-mentioned situation referred to as the INCIDENT, should initiate the following steps:

II. WHAT INCIDENTS SHOULD BE REPORTED

  1. Any Chemical Release (spill, explosion, leak, etc.) capable of affecting others outside your immediate area.
  2. Any Chemical Release impacting the environment (sewers, HVAC, water systems, atmosphere).
  3. Planned or accidental Release of Hazardous Waste or its constituents or air, soil, or water anywhere on campus.
  4. Any of the over 300 Toxic Chemicals in Appendix C.


III. INFORMATION NEEDED FOR REPORTING AN INCIDENT

  1. Exact type of incident (chemical spill or leak, fire, explosion, odor)
  2. Exact name of the chemical(s)
  3. Amount of chemical(s) involved
  4. Building name
  5. Room number
  6. Area and Location if not in the building
  7. Describe incident (amount of spill or leak, suspicion of injury or trapped person)
  8. Your name and where you can be reached
  9. Any additional information regarding safety
  10. Vacate the area

IV. EVACÐãÉ«¶ÌÊÓƵTION PROCEDURE

  1. Activate the nearest fire alarm (alarms are located near exits).
  2. If flammable vapors are present, University Faculty/Staff will direct evacuation and check rooms for occupants.
  3. Close all doors.
  4. Join the evacuated group. Your help may be needed by the response team to supply more information.
  5. Do not re-enter the area until authorized by the response team coordinator.

V. NOTIFICATION OF AGENCIES

THE DEPARTMENT OF ENVIRONMENTAL AND OCCUPATIONAL HEALTH AND SAFETY or the UNIVERSITY POLICE DEPARTMENT will notify the appropriate agencies concerning the situation.

These agencies will include, but not be limited to:

NOTIFICATION SHOULD BE MADE BY PHONE, FOLLOWED BY A LETTER

The report must include:

  1. Name and telephone number of contact person at the facility
  2. Name and address of facility
  3. Time, duration, and type of accident
  4. Name and quantity of materials involved
  5. What occurred (how much was released and to what)
  6. Extent of injuries or damage
  7. Potential additional hazards to humans, property, and the environment
  8. How chemical waste was treated on site

The Department of Environmental and Occupational Health and Safety will supervise the cleanup and recovery of small scale leaks and spills. In the event of larger leaks and spills or the presence of a fire or an explosion, the Akron Fire Department has overall control of the activities.

VI. IMMEDIATE ACTION

Measures will be taken to contain and control the emergency using the Emergency Spill equipment located in these areas:

  1. Knight Chemical Laboratory, receiving area near the loading dock
  2. Superior Building, first floor storage room
  3. Safety Trailer, near KNCL and the Computer Center.

ROUTES FOR TRANSPORTING HAZARDOUS MATERIALS

  1. Knight Chemical Laboratory has a Hazmat waste room and is accessible from Chemical Engineering, Biology, and the Institute of Polymer Science, without crossing public streets.
  2. The Institute of Polymer Science has a waste storage facility due to their high volume. The other departments transfer their waste in 5-gallon polyethylene jugs to the Chemistry Department for consolidation in 55-gallon drums.
  3. Waste from Biomedical, Polymer Engineering and Physical Facilities is accumulated at the S. Forge Street small quantity generating site.
  4. Delivery of all purchased hazardous substances is directed to the department issuing the requisition, in original containers, by commercial carriers.
  5. Special deliveries are made by our Special Services, mostly from Central Stores located on Forge Street. The routes used will include South Forge to Hill Street, University Avenue to Wolf Ledges Parkway, in order to service the area identified above.
  6. Major arteries include East Exchange and Wolf Ledges Parkway in the south, South Broadway in the west, East Mill and East Market in the north, and the Expressway Route 8 in the east.
  7. Evacuation will be carried out by the City of Akron.

APPENDIX A

EMERGENCY SPILL EQUIPMENT (cabinet located in receiving area at KNCL)

  1. One (1) SPILL TAMER KIT:
    This kit is to be used for the spill and disposal of hazardous alkali, flammable solvent, acids, and mercury. Each kit contains a spill tamer absorbent, neutralizers for acids and alkalis, mercury tamer, mercury collection bottle, gloves, brush and pan, spare disposal bag, labels and a Safety Handbook.
  2. Twelve (12) SPILL CONTROL PILLOWS:
    Pillows can be used for chemical, biological, and radioactive spills. Pillows completely contain the spill using an inert and highly effective inorganic absorbent.
  3. Two (2) CO2 FIRE EXTINGUISHERS (20 LB)
    Two (2) ABC FIRE EXTINGUISHERS (10 LB)
  4. Two (2) FACE SHIELDS (with head gear and visor)
  5. Two (2) RESPIRATORS (For Organic Vapors)
    Six (6) REPLACEMENT CARTRIDGES
  6. Two (2) Sets RUBBER GLOVES (Chemically Resistant)
  7. One (1) BONDING/GROUNDING WIRE (When transferring chemicals)
  8. Two (2) OSHA RESPONSE SUITS (Includes Attached Hood and Shoe Covers)
  9. One (1) Roll DUCT TAPE (For Emergency Repairs)
  10. Ten (10) PLASTIC BAGS (For Holding Use)
  11. One (1) SAFETY FILL VENT AND FUNNEL (For Emergency Transfer)
  12. One (1) SELF-CONTAINED BREATHING APPARATUS (Supplies Oxygen)
  13. One (1) Roll of Caution Tape

All of the above equipment is kept in fireproof safety cabinets which are labeled:

"EMERGENCY SPILL EQUIPMENT."

The cabinets and contents are checked weekly as part of the facility drum and safety equipment report.

APPENDIX B

EMERGENCY EVACÐãÉ«¶ÌÊÓƵTION ROUTES

KNIGHT CHEMICAL LABORATORY

Emergency evacuation should be via the corridors and stairwells leading to the lighted exits at the north and east ends of the building. Alternate evacuation routes are to be via the 3rd floor walkway bridge leading toward the Auburn Science Center, and via the 1st, 2nd, and 3rd floors leading into Mary Gladwin Hall and exit by the south exit way on the 1st floor.

AUBURN SCIENCE AND ENGINEERING CENTER

Emergency evacuation should be made via the corridors and stairwells leading to the lighted exists at the east end of the building on the 1st floor. Alternate emergency evacuation should be made via the 1st floor walkway leading into the 3rd floor junction of Mary Gladwin Hall and Knight Chemical Laboratory. Evacuation should be made via the stairwells leading to the 1st floor exit on the north and east sides of Knight Chemical Laboratory or via the stairwell leading to the 1st floor south exit of Mary Gladwin Hall.

WHITBY HALL

Emergency evacuation should be made via the corridors and stairwells leading to the lighted exits at the north end of the building on the 1st floor. Alternate emergency evacuation should be made via the 1st floor stairwell and walkway leading into the 2B level of Auburn Science & Engineering Center. Evacuation then should be made via the 1st floor main entrance of the Auburn Science & Engineering Center.

PHYSICAL FACILITIES OPERATIONS CENTER

Emergency evacuation should be made via the corridors and stairwells leading to the lighted exits on the 1st floor, east side of the Physical Facilities building. The alternate emergency evacuation route should be via the stairwells leading to the lower level of the facility and exit through the rear (west) or north lighted exit ways.

OLSON RESEARCH CENTER

Emergency evacuation should be made via stairwells and corridors leading to the lighted exits at the northwest and the southeast corners of the building. Occupants of the east and south portions of the building should evacuate using the southeast exit and those occupying the west and north sides should use the northwest exit. Once outside, all evacuees should keep at least 50 feet away from the building in accordance with the University evacuation policy.

FOLK HALL

Emergency evacuation should be made through all clearly marked doors and lighted exits since this is a one-story building. Occupants leaving the building through the north side should proceed toward East Exchange Street, those using exits on the east side should proceed to Gallucci Hall, and those using the south doors should proceed toward Wheeler Street. The main idea is to get away from the building and wait for further instructions before returning to Folk Hall. Once outside, all evacuees should keep at least 50 feet away from the building in accordance with the University evacuation policy.

  1. West Tower occupants will exit through the cul-de-sac and proceed toward Guzzetta Hall or Robertson Dining Hall.
  2. East Tower occupants will exit on the Whitby Hall side and proceed toward ASEC or use the bridge to ASEC.
  3. During inclement weather, Guzzetta Hall, E.J. Thomas or Robertson Dining on one side, and KNCL, Whitby, ASEC, and Ayer Halls will house evacuees. In good weather, they will stay outside beyond the 50 feet limit at the discretion of the first responders.
